The Freedom to Speak Up Review
The Freedom to Speak Up Review is an independent review into creating the open and honest reporting culture in the NHS. The review was announced on 24 June 2014 by the Secretary of State for Health and is led by Sir Robert Francis QC. The report was published on 11 February 2015.

The Francis Report
The Francis Report came out of a public inquiry led by Robert Francis QC, established to examine why serious failures in care at Mid-Staffordshire NHS Foundation Trust before 2009 were not acted on sooner by the various responsible organisations.
The report was published on the 6 February 2013. The Government responded to the report's recommendations on 19 November 2013.
