This questionnaire was sent out to gaols and houses of correction around England and Wales as part of the evidence gathering of the 1835 Select Committee of the house of Lords to enquire into the state of the gaols and houses of correction in England and Wales.  A copy of the completed questionnaire was returned to Parliament, and the data contained features in the Report of the Select Committee.
